UKAT accepts referrals from statutory and third-sector partners across England and Wales.

Four-step referral process

  1. Step 01

    Submit Referral

    Complete the form and attach supporting documents.

  2. Step 02

    Assessment

    Telephone or face-to-face, typically within 24–48 hours.

  3. Step 03

    Medical Review

    Clinical suitability and pathway confirmed.

  4. Step 04

    Funding & Admission

    Confirmed with the referring authority.

Checklist

What to include in your referral

For all referrals

  • Referrer name, email, phone, organisation and role
  • Client name, date of birth and contact details
  • Type of treatment required (detox, rehab or both)
  • If detox: type required (alcohol, drug or both)
  • GP summary
  • Details of substances involved
  • Mental health concerns and current support
  • Safeguarding or risk concerns
  • Funding status
  • Preferred centre (if known)

Additional documents by referral type

Alcohol / Benzodiazepine detox (within 3 months)

  • Liver function test
  • Full blood count
  • Coagulation profile if bruising/bleeding
  • U&Es if kidney problems

If CMHT / mental health involvement

  • Up-to-date care plan and risk assessment
  • CPN and Psychiatrist contact details

If comorbidities

  • GP letter of consent and support
  • GP medical summary

If extensive criminal history

  • Copy of pre-cons

If DVTs or abscesses (drug referrals)

  • Current wound management plan
